Current Patient
• For Current/Established patients ONLY.
• For a “problem or sick visit” or “follow up” appointments.

Current Patient Annual Physical
• For Current/Established patients ONLY.
• Will cover the Annual Wellness Exam ONLY.
To help expedite your visit, please bring the following items:
1.) Insurance Card
2.) Photo ID
3.) Current Patient Annual Physical forms (complete prior to appointment)

New Patient
• For New Patients ONLY.
• For a “new problem or sick visit” 
• This appointment is NOT to be used for New Patient Annual Physicals
o If you need a New Patient Annual Physical, please call our office at 303.268.1571
To help expedite your visit, please arrive 15 minutes prior to your scheduled appointment time and bring the following items:
4.) Insurance Card
5.) Photo ID
6.) A list of current medications
7.) New Patient Registration Forms (completed prior to appointment)
8.) Patient HIPPA Acknowledgment & Disclosure Consent (Print) (completed prior to appointment)
